Diagnostic and therapeutic hysteroscopy in Ternopil

Hysteroscopy is an informative minimally invasive method of diagnosis and treatment of gynaecological pathologies. It is performed to visualize and assess the state of the uterine cavity, endometrium, cervical canal and adjacent structures, as well as to take biological material for further histological analysis and removal of small benign neoplasms.

It is carried out with the help of a rigid optical device called hysteroscope and makes it possible to establish the correct diagnosis as well as to perform a small surgery, if necessary.

The method is safe, has practically no contraindications. The entire procedure lasts only 20 minutes in the case of diagnosis, and up to 40 minutes in the case of surgery.

Types of hysteroscopy

Diagnostic hysteroscopy is prescribed when ultrasound and laboratory examinations are uninformative. Indications are:

•     violation of the menstrual cycle;

•     long heavy menstruations;

•     uterine bleeding;

•     painful sensations during sexual contact;

•     unexplained cause of female infertility;

•     several episodes of miscarriage;

•     preparation for the IVF, ICSI procedure;

•     suspicion of a malignant tumour;

•     chronic pelvic pain;

•     uterine bleeding during menopause.

The method allows to detect:

•     anomalies of uterine development;

•     endometrial hyperplasia;

•     myoma, in particular submucosal one;

Curative hysteroscopy (hysteroresectoscopy) is a minimally invasive gynaecological surgery. It is prescribed for a wide range of uterine pathologies, including:

•     myoma of the uterus;

•     endometritis;

•     endometriosis;

•     adenomyosis;

•     polyps;

•     endometrial hyperplasia;

•     atresia of the cervical canal, etc.;

•     removal of foreign bodies (spiral).

Preparation

Diagnostic and therapeutic hysteroscopy is performed after an examination prescribed by a gynaecologist. The examination includes:

•     blood and urine analysis;

•     swabs from the vagina and cervical canal;

•     test for HIV infection, syphilis, hepatitis;

•     fluorography, ECG;

•     ultrasound of the pelvic organs.

Hysteroscopy is performed a few days after the end of menstruation. Three days before the procedure, you should refrain from intimate contacts.

Stages of the procedure

Surgical hysteroscopy takes place in the surgery room on a gynaecological chair under anaesthesia. This is usually a medicated sleep. An anaesthesiologist is present during the surgery.

1.   After aseptic processing, the cervical canal is carefully expanded, a hysteroscope is inserted into the uterus.

2.   A thorough sequential examination of the walls of the uterus and adjacent structures is carried out.

3.   Based on the imaging data, an assessment is made, a diagnosis is established, and the necessary manipulations are performed (scraping, resection of polyps, taking biological material from the problem area, etc.)

Hysteroscopy is used to remove small polyps in the uterine cavity. These are benign neoplasms on a pedicle, which are often the cause of infertility, bloody discharge, failure of the menstrual cycle, metrorrhagia and pain during sexual intercourse and pose a certain threat of malignancy. They are removed on an outpatient basis using the hysteroscopic method.

To prevent recurrence, the bed of the polyp is coagulated (cauterized) with the help of a laser. The technique is atraumatic, which is of great importance for pregnancy planning.

Recovery

A few hours after the surgery, the patient receives recommendations and can go home. Slight soreness in the lower abdomen and small bleeding are a normal reaction of the female body to medical intervention. Normally, these symptoms pass on the 2nd-3rd day. For a quick recovery, it is necessary to follow the doctor's advice and exclude:

•     physical overstrain, lifting heavy objects;

•     sexual contact within 4 weeks;

•     hypothermia and hot procedures (bath, sauna);

After receiving the results of the histological examination, the doctor makes a final diagnosis and prescribes appropriate treatment to avoid recurrence and cure the cause of the development of a specific gynaecological pathology.
